Information collected
The consultation form asks for your first and last name, email address, phone number, session interest, preferred contact method and acknowledgment of this Privacy Notice. The form does not request medical information. Please do not submit diagnoses, medications, injuries, symptoms, pregnancy information or other health details.
Like other hosted websites, the hosting infrastructure receives technical request information needed to deliver and protect the site, such as an IP address, request date and time, requested page, browser headers and similar network information.
How information is used
Consultation details are used to respond to your request, discuss availability and session preferences, and maintain ordinary business correspondence. Submitting the form does not consent to promotional texts, automated calls or addition to a marketing list.
Submission, email and hosting providers
The consultation form is processed by FormSubmit, which forwards the submitted information to Be Well’s Gmail account. FormSubmit states that it retains form submissions for 30 days. Google processes the delivered email under its applicable terms and privacy practices. The website is deployed through OpenAI Sites using Cloudflare-based hosting infrastructure.
Analytics and cookies
The current project does not include custom analytics, advertising pixels or marketing-cookie code. Hosting, security and form-processing services may use technical mechanisms needed to operate and protect their services.
Recipients and disclosures
Consultation information is processed by FormSubmit and delivered to Be Well through Google’s Gmail service. Technical request information is processed by the hosting providers described above. Be Well does not sell consultation information or transmit it to advertising services.
Retention and deletion
FormSubmit states that it retains submissions for 30 days. Copies delivered to Be Well’s Gmail account remain subject to Be Well’s email-account settings and deletion practices. You may request access, correction or deletion by emailing bewellhouston@gmail.com. Be Well will evaluate the request subject to information it controls and any applicable recordkeeping requirements.
Security limitations
No internet transmission, form-processing service, email service or storage system can be guaranteed completely secure. Do not submit sensitive medical, financial or identification information through the consultation form or ordinary email.
Updates and contact
